Benefits

Outline of the health and benefits plans offered by the Company:
• Company paid Malpractice coverage subject to meeting the underwriting guidelines.
• Medical insurance through AETNA plans: 3 Open Access POS plans with out of network
benefits
• Healthcare Reimbursement Arrangement (HRA) or Health Savings Account (HSA):
Employer contribution $1,500 per employee per year that is eligible 1/1/2023. The
amount will prorate depending on your eligibility date.
• Mileage reimbursement at $0.30 / mile.

Additional Employee Funded Benefits:
• 401(k) Plans (Traditional plan with pre-tax benefits and a Roth 401k post tax)
• Short-term Disability. Subject to medical underwriting.
• Long-term Disability. Subject to medical underwriting.
• Life & AD&D for self, spouse and/or children. Subject to medical underwriting if over the
Guaranteed Issue amount of $200,000.
• Dental (DHMO plan only in FL or a PPO plan)
• Vision
• GAP insurance: Up to $5000 for inpatient hospitalization. $2500 for outpatient services.
$1000 ambulance. Must be enrolled in medical insurance with Vohra or elsewhere.
• FSA for Healthcare Expenses up to $3,050 for 2023 for those that elect the AETNA OAMC
$5000 and AETNA OAMC $1000 Medical plans and FSA Dependent Care Assistance up to
$5,000 per household.
• HSA for employees that elect the AETNA OAMC HSA medical plan. $2,350 (individual) or
$6,250 (family) can be contributed pre-tax annually. Additional, $1000 catch-up
contribution for those age 55 or over.
• Limited Purpose FSA for those that elect the AETNA OAMC HSA medical plan
• Accidental and Hospital plan
• Critical Illness plan
Eligibility for benefits begins when your permanent weekly encounter average exceeds 50 for
the prior 4 weeks. Enrollment is the first of the month of the following month.
